NOTICE OF PRIVACY PRACTICES

This notice describes how medical information about you may be used & disclosed & how you can get access to this information. Please review this carefully.

YOUR PROTECTED HEALTH INFORMATION (PHI)

Each time you have contact with DSDAS, LP. dba Artisan Medspa, Laser & Wellness & Artisan Aesthetics for delivery of healthcare, a medical record is prepared. This record, maintained in written, oral, or electronic format, contains presenting signs/symptoms because of exam & test. Your medical record(s) is the property of DSDAS, LP. dba Artisan Medspa, Laser & Wellness & Artisan Aesthetics but you have certain rights to restrict some of the use or disclosures of the-information in your record. However, DSDAS, LP. dba Artisan Medspa, Laser & Wellness & Artisan Aesthetics., has the right to disclose the information in your medical record in the process of providing treatment, receiving payment & performing other regular health options such as: Documenting & describing the care you received for legal purposes, communications with other healthcare providers who may be involved in your care, providing information to government & public health entities, billing & verification of services provided to you and/or routine healthcare operations for quality improvement & assessing health care provider competence.

HOW WE MAY USE & DISCLOSE YOUR PROTECTED HEALTH INFORMATION (PHI)

Healthcare Delivery & Treatment-Information obtained from you by a Physician, nurse or other healthcare professional is documented in your record & used for the assessment, evaluation, diagnosis & treatment of your medical condition(s). Following treatment, this information may be provided to other healthcare professionals who may be involved in your care, such as other Physicians & healthcare providers.

Billing & Payment-Your PHI is used to justify the level of care delivered to you & the charges incurred for services. This information generally accompanies the bill & is sent to our payers.

Other Healthcare Operations-DSDAS, LP. dba Artisan Medspa, Laser & Wellness & Artisan Aesthetics may disclose your PHI to other individuals & businesses to perform day-to-day operations. These individuals include but are not limited to vendors and/or contractors used for billing & claims, disease management & quality improvement initiatives as well as services such as laboratories & radiology.

Reminders & Treatment-You may be contacted by a member of our staff to provide you with information we feel is useful or helpful to you based on your PHI. For example: you may receive a call to check on your treatment status, suggest an alternative or provide you with information on treatments you are already receiving.

Other Uses & Disclosures-Your information may also be utilized or disclosed to communicate with or notify your family members or other responsible health care providers.

Employment Uses & Disclosures-Your information also may be used to disclose information to communicate with or notify your employer, school, or other person(s) or entity responsible for payments for care or treatment you received. If the treatment provided for you is related to your employment, such as a screening physical or occupational medical care, DSDAS, LP. dba Artisan Medspa, Laser & Wellness & Artisan Aesthetics will disclose your PHI to your employer for such purposes.

Other uses & disclosures of PHI are not permitted or required by law & will be made only with your written authorization. You may revoke your authorization at any time provided that the revocation is in writing to Artisan Medspa, Laser & Wellness & Artisan Aesthetics.

YOUR RIGHTS CONCERNING PROTECTED HEALTH INFORMATION (PHI) Except as otherwise provided by law, you have the right to:

Receive a paper copy of this NOTICE OF PRIVACY PRACTICES. We reserve the right to change the terms of our notice at any time. Upon your request, we will provide you with a revised notice by mail or on your next visit.
Receive confidential communications of PHI if a request is submitted to DSDAS, LP. dba Artisan Medspa, Laser & Wellness & Artisan Aesthetics in writing.
Inspect & copy PHI or records about you in a designated record set as long as the PHI is maintained in the record set.
Designate personnel to represent the use or disclosure 6f PHI.
DSDAS, LP. dba Artisan Medspa, Laser & Wellness & Artisan Aesthetics can amend PHI or records about you in a designated record set; however, they are not required to change information if it is deemed to be inaccurate.
Receive an accounting of disclosures of PHI. You may request a listing of the disclosures made by us about you for reasons other than treatment, payment, or healthcare operations.
Request that DSDAS, LP. dba Artisan Medspa, Laser & Wellness & Artisan Aesthetics restrict uses or disclosures of your PHI. Though they are not required to agree to a restriction to the extent that it does agree with your request, DSDAS, K.P. dba Artisan Medspa, Laser & Wellness & Artisan Aesthetics may not use or disclose the protected PHI in violation of the restriction unless the information is needed to provide emergency treatment or is otherwise permitted or required by law.

DSDAS, LP. "Dba Artisan Medspa, Laser & Wellness & Artisan Aesthetics is required by law to maintain the privacy & confidentiality of your protected health information & to provide you with a notice of its legal duties & privacy practices with respect to such health information. They are also required by law to abide by the terms of this NOTICE OF PRIVACY PRACTICES & allow you to review & receive a copy of this notice. If you believe your privacy rights have been violated, you may submit a written complaint to the U.S. Department of Health and Human Services describing in detail the way you feel your privacy rights have been violated. DSDAS, LP. dba Artisan Medspa, Laser & Wellness & Artisan Aesthetics will not retaliate against you in any way if you choose to file a complaint.
